Habilitative Program Administrator

The Habilitative Program Administrator (HPA) will serve as a member of the Washington State Clinical Team, creating and overseeing habilitative plans for adults with intellectual/developmental disabilities in the Supported Living Program in Olympia.Use information and data collected through direct observation and meetings with individuals supported to develop Functional Assessments and Positive Behavior Support Plans (PBSPs)Develop Individual Support Plan Programs for individuals on caseloadTrain team members on plans and program implementation Partner with program directors to implement support plan programs Using electronic data collection system (Therap), review data and compile reports for other members of support team on behavior and habilitative goal progressAssist in the creation and maintenance of tracking spreadsheets for relevant due dates and person-centered planning meetingsKeep up to date on all relevant state policy changesMaintain open lines of communication with stakeholders outside the organization, i.e. guardians and DDCS partnersAttend support planning meetings as required
