Job Summary:The Placement Manager plans, directs, manages, and oversees the Health Plan benefit, clinical service delivery and business performance to assure the safe transition of patients to Skilled Nursing Facilities (SNF), Acute Rehabilitation and Long-Term Acute Care Hospitals (LTACH). Directs and oversees the provision of high-quality care at contracted facilities and agencies, which meets all regulatory, accreditation, licensure and certification requirements, and the goals and missions of the organization.Essential Responsibilities:
Directly and through subordinates, plans, organizes, directs, and controls the operation of assigned areas (e.g., general administration, patient care, professional services and/or a combination of these and other services). Contributes toward the overall development of programs and services to provide for the needs of members, physicians and staff. Provides strategic leadership to management to ensure equitable distribution of resources and delivery of high quality, cost-effective healthcare and efficient management of staff in accordance with goals, objectives, policies, and applicable regulatory agency rules and regulations. Co-leads unit-based teams (UBTs). Ensures the UBTs continuously focus on identifying and implementing ways to meet/exceed quality, service, and access targets and implement regional initiatives. Collaborates with other departments to ensure smooth transitions and coordination of care.
Responsible for the day-to-day business operations for contracted SNF and ICF facilities by facilitating established department goals, implementation and monitoring of service, quality, utilization standards and regulatory requirements as well as appropriate policies, standards, practices, legal and regulatory requirements and government/accrediting agencies. Evaluates, plans and organizes service and/or care delivered by staff. Oversight and responsibility for department budget, staffing, expenditures and payroll. Responsible for hiring, performance management and terminations as well as mentoring and development of staff. Partners with Health Plan and Permanente to identify and establish programs and practices which are cost effective and provide quality service to members, staff and physicians.
Assures the delivery of quality care by collecting, analyzing, trending and monitoring data and incorporating the data in the quality improvement plan. Presents quality and utilization data to leadership both within and outside of the department in partnership with the Senior Manager.
Maintains personal and professional growth of self and others by participating in educational offerings and professional organizations.
Provides constructive feedback and effectively manages conflict. Follows up on feedback and reports problems as well as positive comments. Maintains an environment which recognizes and values differences in staff, physicians, patients and communities. Collaborates with other organizational nursing programs. Routinely rounds on staff to communicate key messages and identify concerns and issues.
Basic Qualifications:Experience
Minimum two (2) years of general management experience in a complex, multi-faceted healthcare setting
Minimum two (2) years of experience in financial management, budget development, program development, quality improvement and project management
Minimum one (1) year in a Home Health/Hospice or long-term care and/or community setting or Health Plan operations
Education
Bachelors degree in Nursing, Physical Therapy, Occupational Therapy or related field OR four (4) years of experience in a directly related field.
High School Diploma or General Education Development (GED) required.
License, Certification, Registration
Physical Therapist License (Oregon) required at hire OR Compact License: Registered Nurse required at hire OR Registered Nurse License (Oregon) required at hire OR Occupational Therapist License (Oregon) required at hire
Occupational Therapist License (Washington) required at hire OR Registered Nurse License (Washington) required at hire OR Physical Therapist License (Washington) required at hire

Additional Requirements:
Thorough clinical and administrative knowledge of geriatrics as well as short and long-term care
Competency in program planning and development, financial analysis, budget preparation
Thorough knowledge of continuous quality improvement and its application in the healthcare setting
Thorough knowledge of Medicare regulations/standards governing skilled nursing facility care and intermediate care facilities
Demonstrated customer-focused service skills
Ability to effectively utilize computerized medical records
Ability to collaborate with regional and national programs to leverage best practices
Proven leadership skills
Working knowledge of quality, risk management and infection control principles
Excellent written and verbal communication skills; analytical problem-solving skills; well-developed project management skills; competent quantitative skills
Proficiency in the use of applicable computer software (e.g., MSWord, Excel, Outlook)
Willingness to work in a Labor/Management Partnership environment
Preferred Qualifications:
Minimum two (2) years of change management experience in a healthcare setting
Minimum two (2) years of managerial experience within Kaiser Permanente or similar Health Maintenance Organization
Previous recent RN nurse leadership experience in a primary care setting
Previous experience within Quality/Utilization Management
Demonstrated understanding of Nurse Practice Act, Labor Law, all related contracts, workmens comp, HR policies and procedures, OSHA, Employee benefits, National Compliance, Labor Management Partnership
Masters degree in Nursing, Health Care Administration or Business Administration.
