OVERVIEW: Under the supervision of the Clinical Program Manager or Lead/Urgent Response Supervisor, the Urgent Response Coordinator (URC) provides urgent response to youth and caregivers as the first point of contact to those seeking assistance in the urgent response department. This involves phone triage, in-person response, de-escalation, and follow up care to ensure increased safety, stability, and linkage to community, health, educational, social, and mental health resources.

DUTIES:

Provide assessment, triage, and crisis response to those seeking support through the various program contact methods (phone, text, chat, etc.).

Coordinate needed response to client/caregiver crisis and involve additional roles (therapist, advocates).

Provide short-term follow up services to client/caregiver to further implement techniques from developed safety plans, teach basic coping skills, and facilitate linkage to other providers and community resources.

Take responsibility for initial safety plan development or escalate to Therapist when appropriate and implement plan with youth and caregivers.

Provide in-person support as needed to stabilize crisis and support youth and families.

Facilitate a warm hand-off to county mobile response programs as needed.

Complete required documentation for each client/family interaction in the EHR within required deadlines.

Complete notifications to providers, CDSS, etc. per program requirements.

Provide resources and referrals to youth and caregivers as needed.

Ensure thorough and accurate data entry in the identified call center software.

Complete all documents for identified program, in a timely manner.

Attend external meetings to support client/caregiver such as emergency CFTs, etc. as appropriate.

Assist in the ongoing development and evaluation of the service program.

Monitor and ensure effective linkages with community and agency service providers.

Participate in training activities as directed.

Participate in outreach activities and youth and caregiver events as directed.

Establish and maintain positive and effective working relationships with internal staff and external resources.

Adhere to policies, systems and standards of the Sacramento Children’s Home and all applicable programs, agencies and contracts.

Participate in supervision, staff meetings, process reviews and other assigned meetings.

Perform other duties as assigned.

QUALIFICATIONS:

Applicant MUST have at least one (1) year of full-time/equivalent (FTE) experience in a mental health or related field providing direct mental health is required.

In addition to one (1) year experience working in/with community mental health, crisis work, mental health hotlines, seriously disturbed children, adolescents, and families, and experience in engaging parents as partners and working successfully with families from diverse ethnic and cultural groups is also preferred.

Possession of a Master’s Degree or PhD and 2 years of full-time/equivalent (FTE) direct care experience in a mental health setting or a Bachelor’s degree and 4 FTE direct care experience in a mental health setting, or Associates Degree and six years of FTE direct care experience in a mental health setting is highly desired.

Driving is an essential function of this job. As such, employees in this position will be required to furnish their own vehicle to be used in the course of employment. A valid driver’s license and minimum insurance coverage is also required. Must be at least 21 years old.
