Role OverviewAre you passionate about making a real difference in the lives of young people and their families? Join our dedicated team as a Case Manager, where you’ll play a crucial role in supporting and guiding residents through their journey with our programs. Key ResponsibilitiesYouth and Family Engagement: Build strong, supportive relationships with youth and their families, adapting to their needs in transitional and aftercare settings.
Client Coordination: Swiftly review and process referral packets, coordinate interviews, and ensure seamless communication with clinical teams and external contacts for client acceptance.
Intake and Documentation: Complete comprehensive intake paperwork and assessments within strict timeframes, including immediate needs evaluations, safety plans, and mental health status forms.
Treatment Planning: Develop and update treatment plans, including 72-hour and 30-day plans, and coordinate treatment team reviews to ensure ongoing, effective care.
Assessment and Compliance: Conduct and document necessary assessments (CANS, YRAT) within specified deadlines, and maintain records in line with Caring Together, EEC, and Youth Services standards.
Collaboration: Engage with DCF, school personnel, and other agencies to provide updates, share progress, and support the resident’s treatment goals.
Quality of Life: Oversee and ensure high standards for residents’ dietary, hygiene, and overall well-being.
Program Support: Actively participate in meal preparation, sanitation, and scheduling of harm reduction and independent living skills groups.
Leadership: Serve as a role model for both residents and staff, demonstrating professionalism and positive leadership.
On-Call and Transportation: Manage on-call responsibilities, transport youth when necessary, and ensure all appointments and aftercare outreach are completed effectively.
Training and Development: Facilitate in-house training sessions, create program groups, and maintain up-to-date training logs.Essential QualificationsEducation & Experience: Bachelor’s degree in psychology or a related field, with at least 2 years of experience in a residential setting.
Skills: Exceptional communication, organization, and problem-solving skills. Ability to adapt to a changing environment and handle multiple priorities.
Certifications: Must be willing and able to obtain the following certifications: CPR/First Aid, Crisis Dynamics (including Physical Restraint), Medication Administration Program (MAP), HIPAA, OSHA, Ethics and Boundaries, Trauma-Informed Care, and Collaborative Problem Solving.
